This takes "blame the victim" to a whole 'nother level. Very tragic. Accidents will happen when industrial equipment is run by a single operator. What's missing is what all the other employees were doing while Mr. Contreras-Benitez was running the chipper. The fine that OSHA is levying is so minimal, the company should gracefully pay it and let the man rest in peace.
