Building inspector fell after climbing guard rails

A City of Palo Alto-contracted building inspector who plunged to his death down an elevator shaft had climbed on two guardrails that gave way, according to an investigative report.

Our sincere condolences to the wife and two children of Gary Collins.

Gary Collins will be missed. It is unfortunate that this tragic accident has the City of Palo Alto Chief Building Official and other city management excusing themselves for not reporting this incident to the public, or offering the family public condolences, because there is no "city policy" regarding tragedies such as this accident. Even when contacted by local media, City Manager Keene fails to respond accordingly and offers a "no comment" explanation as to why this accident was not publicly disclosed and still offers no public condolences to Collins family members. There appears to be a deep dysfunctional disconnect within the current city management and their ability to communicate on a normal human level. What a pity.

Dear Izio,

On November 2nd 2011, Collins was linked to the divorce scandal in Contra Costa Called CNET or Dirty DUI.

My neighbor was former Officer Tanabe, his friend was Chris Butler, and this crew is likely behind suspicious deaths in the area. I don't think we're going to know the truth but his blonde hair will be critical in another high profile murder case.
